The Office: The Right Yang Wei

  • Normal physiological pulse is faint
    • Clearly defined pulse that is superficial, round, and not strong or weak
  • Deep and Blocked Pulse
    • Very difficult to feel, often beginners will miss it
    • Phlegm and blood stasis in lung, can also have some heat
    • Usually a dry problem
    • Herbs
      • Xing Ren – 15
      • Mao Dong Qing – 15
      • Di Long – 15
      • Xie Bai – 15
      • Ning Sou San – 40
    • With Wheezing
      • Add Ding Chuan Tang – 20
      • Reduce Ning Sou San – 20
  • Deep and Pounding
    • Phlegm heat and blood stasis in lung
    • Will also have dryness due to heat
    • Herbs
      • Xue Fu Zhu Yu Tang – 20
      • Di Long – 15
      • Xie Bai – 15
      • Mao Dong Qing – 15
      • Lung PH – 45
  • Wiry
    • Can be thin and tight, or have a jump
      • Wind attacking the lung
      • A wider pulse with a jump indicates wind heat
        • Ren 22 will be hot
        • Herbs
          • Sang Ju Yin – 20
          • Lung PH – 20
          • Ning Sou San – 20
          • Herbs to stop coughing
            • San Bai Pi – 15
            • Ting Li Zi – 10
            • Xing Ren – 15
      • A wiry thin tight pulse indicates wind cold
        • The patients face, and Ren 22 will also be cold

          • Xian Qing Long Tang – 30-40
          • Gui Zhi Tang – 20
          • Zi Wan – 15
          • Bai Bu – 15
          • Moistener (Use one of the below)
            • Qing Zao Jiu Fei Tang – 10

      • If Pulse is weak
        • Add Huang Qi – 15-20 or Yu Peng Feng San – 15- 20
        • Otherwise cold will linger!
  • Strong in all three levels
    • Rounded and pounding in all three levels
    • Excess fire
    • Herbs
      • Fu Hai Shi – 15
      • Xin Ren – 15
      • Sang Bai Pi – 15
      • Ge Qiao – 15
      • Bai Hu Tang – 20
      • Lung PH – 40
      • Qing Fei Tang – 20

The Office: Major vs. Minor Patterns

  • Major problems are the predominant patterns that the patient has.
  • Minor problems are secondary to the larger pattern.
  • How do you tell the difference? By the pulse depth.
  • For example, a low and pounding pulse:
    • Major: Damp
    • Minor: Heat
    • Why?
      • If the major problem was heat, it would take the pulse to the surface and dry the damp.
      • The major problem here is dampness, which drags the heat and the pulse down.
    • Use this same logic to determine how to treat cases:
      • Use most of your herbal formula for the major problem, usually 80%.
      • Use a smaller amount the formula to go after the secondary problem(s) usually 20%.

The Office: Common Right Cun Pulses

  • There are 3 typical right cun pulses
    • Too high and pounding
    • Too high and wiry
    • Too deep and blocked
  • Too high and pounding
    • Heat/Fire
    • Treatment: Put out the fire/ clear heat
    • Herbs
      • AST – 20
      • Herbal ABX – 15
      • Huang Lian Shang Qin Wan – 15
      • Seattle 1 / Clear Heat / Huang Qin – 15
      • Fu Hai Shi – 15
      • Ge Qiao – 15
  • Too High and wiry  pounding
    • Wiry Thick, and high
      • Wind Heat
      • Treatment: Expel wind, clear heat
      • Acute Cold or allergy problems
      • Herbs
        • Allergies (allergies always have itching)
          • Sang Ju Yin – 50
          • Cang Er San – 50
        • Or
          • Sang Ju Yin – 33
          • Cang Er San – 33
          • Xin Yi San – 33
        • With itching use
          • Ku Shen
          • Ba Ji Li
            • Especially for itchy eyes
          • Chan Tui
          • Bai zhi
            • Especially for itchy nose
          • Feng Fang
          • Jing Jie
        • Sneezing and runny nose, increase the Sang Ju Yin
  • Deep and blocked
    • Sinus congestion/ infection
    • Dried up phlegm with blood stasis
    • Treatment: Move blood, transform phlegm, moisten dryness, astringe fluids
    • Herbs
      • Di Long – 20
      • Herbal Abx or Wu Wei Xiao Du Yin – 15
      • Astringents – 15
      • Clear heat /Seattle 1 – 15
      • Huang Lian Shang Qin Wan – 15
      • Resolve EM – 40
    • One problem with is approach is it very drying
    • Use sweet herbs or formulas to moisten fluids
      • Qing Zao Jiu Fei Tang – 20
