The Office: Needling Recommendations

  • Needling the Wrists
    • Start away from the proximal wrist crease and needle towards the fingers to reach the retinaculum
  • Needling Ankles
    • Needle towards the knee
    • Angle for Ki 6 will have to be slightly less to find the point
  • Becoming good at needling the joints at the wrist and the ankle will help you avoid using lots of needles
  • Needles
    • Bob freehands with 32-gauge needles
    • Modern Acupuncture is using 34-gauge needles with tubes
      • Use a guide tube
      • Be careful with bent needle tips
        • Reangling small needles has a high chance of bending the needle tip
    • Do not use a 36-gauge needle
      • Too small for much effect
    • 30-gauge will cause more complaints
      • More likely to bounce off a nerve and disturb the patient.

The Office: Herbal strategy

  • Usually, the worst pulses are in the heart, liver, upper respiratory system or digestive symptoms.
    • Usually, kidney issues are not seen until the patient is past the age of 50 or 60.
  • What you treat first depends on what the patient needs and what they want.
    • For example: If they engage your services to fix their allergies, but also have a heart problem. You need to go after the allergies until they start feeling better. Then slowly start working on the heart.
  • Always explain to the patient what health issues they have, and what you are treating.
  • If there is a liver issue, you must treat that first for 2-3 weeks.
    • They won’t feel better if you don’t treat the liver first.
    • After the liver calms down, leave 20% of the formula for the liver.
  • With patients over the age of 50, once they are in the 70% improvement range. You need to consider adding kidney tonics. Usually 20% of the formula.
    •  If the pulses are high.
      • Add kidney yin tonics.
    • If pulses are low and thin.
      • Add kidney yang tonics.
  • You must help their symptoms/ chief complaint first, so they trust you.
    • Once they trust you, you have a broader latitude to fix their health and focus on the major problems.
  • Never ignore the heart!
